I hope this helps you . Try having ffmpeg directly operate on the input/output files, instead of piping data into and out of ffmpeg:

wish help you to fix your issue Have you checked out the ffmpeg library for PHP? I've worked with it before and it abstracts away all of the complicated command line calls like the one you're trying to run. You can use $movie->getFrame($n) to get the frame object, and $frame->toGDImage() to output it as an image.
For example,

$frame10 = $movie->getFrame(10);
$image = $frame10->toGDImage();

// Now display the image
header('Content-Type: image/jpeg');
imagejpeg($image);

// Or save it to a file

like below fixes the issue That's not how lossy codecs work. Encoding is a tradeoff between CPU time spent on encoding, bitrate, and perceptual quality. With different settings for the same codec, you will get different output.
Even using the exact same settings, feeding the output of x264 back in as input will cause generation loss every time. Some codecs (like prores I think) are designed so generation-loss stops after a generation or two, but h.264 isn't like that. Partly because it has a deblocking filter in the loop, and partly just because of how it has limited accuracy for references.

it should still fix some issue Some ideas:
make sure you have a recent version of ffmpeg because at some point there was a bug which messed up PCR insertion when stream copying

will help you FFmpeg emits its logging messages on stderr.
2> /var/www/html/conversion_server/files/temp/1527058199834e4.tmp captures that to the file specified.

With these it helps I am trying for days to convert .dav file (file generated by dvrs [image recorders]). I have tried several variations with ffmpeg and can not succeed. , Using ffmpeg from dav to mp4
